Disc A includes former site of a graveyard on Long Rifle Road, Goodman-Hershey Family Graveyard, Harnisch Family Graveyard (former site), Herr Family Graveyard, Herr-Kendig-Spahr Graveyard (former site), Herr-Mayer Family Graveyard, Houser Family Graveyard (former site), Kreider-Snavely Family Graveyard, Lampeter Reformed Mennonite Cemetery, Lefever Family Graveyard, Longenecker Reformed Mennonite Cemetery, Rohrer-Mylin Family Graveyard, and Weber-Diffenbaugh Family Graveyard (former site).

Disc B includes Tschantz Graveyard, Willow Street Mennonite Church Cemetery, and Willow Street United Church of Christ Cemetery.
